Wiki Qualifying Use of 96112

We are a family practice. When a child comes in for a well child visit, we have them complete an ASQ developmental screening form. The doctor or nurse practitioner then reviews the form and states in the documentation of the visit whether they have any concerns or not based on the scores of the questionnaire. Does this qualify to use CPT 96112 Developmental Testing? Would appreciate any guidance, thanks!

Here's some guidance from the AAP that may be helpful. It is my understanding that the score(s) must be documented in the note in order to bill these codes.
