I am in the need of a sample query form. I work in a hospital and sometimes need to ask the physicians a query in regards to some dictation clairification on surgery or etc. and would like it to look a little more professional than me just writing it. Does anyone have such a thing or something I could look at to try and adapt to what I need. I have tried going on the internet to look for something but not quite finding what I need or else they want to charge for it. Thanks!! :)
