The ER report states that a pt has an aortic aneurysm but on the radiology report it states that it is an abdominal aortic aneurysm. Can I code it off of the documentation on the X-ray report I should I query the Dr to get the specifics?
 
no you may code form the radiology report for provider coding, for inpatient facility, you may use the radiology report for increased specificity.
