Question about a screening colon?

A patient had a screening colon in 2006 was normal, came back in 2010 for a diagnostic with issues and adenomatous polyps were found. Came back in 2017 for a surveillance and was coded 45380 with a 33 with personal history as the primary diagnosis but was denied and went to the patients deductible. Anyone have any ideas of how to re-file or do we need to do an appeal? Patient has BCBS. Has anyone else seen this happening?
