If patient received 150mg of medication but it is taken from a 200mg vial what would you bill. Would you bill only what you gave or would you bill what you took from?

I believe you bill what was given if the remaining amount in the vial can be used for another patient. If not, you bill the 150mg, then the remaining 50mg on a separate line with modifier JW for unused medication since the remaining amount in the vial cannot be used for another patient. Patient did not recieve the whole 200mg, only 150mg, so it would not be correct to bill them for the whole vial. Your insurance payers may or may not reimburse the unused portion at a lower rate.
