I was told whenever a dr preforms a procedure in a hospital, ex: cholecystectomy, the dr always uses the modifier 26 with the proper cpt code. Is that correct??

that is incorrect you justcode the procedure code. The 26 modifier is for when you perform procedure in the facility and the facility owns the equipment needed such as radiology procedures, not general surgery.

The surgery code would not have a 26 modifier, the S & I code would. Look in the Physician Fee Schedule Database for the PC/TC indicator 1.
