I had a question I am a new coder and this question came up on a test that I took.
What modifiers can be used when an E/M code is billed with a procedure?
 
E/M Modifier with Procedure

-25 indicates a seperately identifiable E/M with a procedure, it is added to the E/M code,
(99213-25). The documentation must support both services and indicate medical necessity.
 
-24 Unrelated E&M Service by the Same Physician During a Postoperative Period
-25 Significant, Separately Identifiable E&M Service by the Same Physician on the Same Day of the Procedure or Other Service
-57 Decision for Surgery
