if we are never to assume...

I see chronic cholecystitis nowhere in the documentation. I do see multiple episodes of severe pain in the midepigastric area. Further down in the op note, I see acute cholecysitis?

Is this an error and should it be acute or do I assume it is chronic due to multiple episodes of severe pain? Thank you for your help.
