Wiki question on coding low grade lesion of cervix

I am billing for pathology, and have a gyn specific question because I am getting conflicting info...

If a patient has a cervical pap that is abnormal low grade squamous lesion I am coding R87.612. This dx specifically says smear.

now the pt has cervical surgical biopsy, finding of low grade lesion, I am coding N87.0 for mild dysplasia.

Am I right or wrong? Looking to see what other people are doing in this scenario. TIA!
 
I would say that is incorrect. Coders are not allowed to extrapolate a dx. I would not code dysplasia unless stated as dysplasia. If you go to the index Abnormal>Specimen>Female Genital Organs - (smears) is in brackets - meaning nonessential. I personally would stick with R87.612
