Hi,

This is a very interesting question, but according to Cpt code book.0282T -includes removal at the conclusion of Trial period, includes imaging guidance and many other details.
but if you just observe in the same page 0282T-0285T should not be reported in conjuction with 64550-64695 and many more codes.
This is all i can try but if anybody gives more information it will be great too.
