Can Z76.89 be used as a primary diagnosis to establish care? If not, what diagnosis can we use to establish care when provider does not complete a preventative visit.

According to ICD-10-CM it is unacceptable as a primary diagnosis. Were any conditions discussed? Was any treatment provided? If so, code that. Meet & Greet visits are tricky; there's really no medical necessity if conditions, illnesses or preventive care are not addressed. Those visits should either be no charge, or billed to the patient, since, per many payer contracts you are not supposed to provide non-medically necessary services.
