R19.7 (diarrhea) could be used as a primary diagnosis.

As with any sign or symptom diagnosis, you'd only use if the provider hadn't confirmed a related definitive diagnosis. For example, if the provider determined that the patient had a viral gastroenteritis, you'd use the appropriate code for that and drop the R19.7.

R19.7 can be billed with an E/M visit. The exact level would depend on the medical decision making (or time).
