Does anyone know if a specialist can bill for a mantainance visit, like a well visit or yearly exam. I think no, only a PCP can bill for that, I want to make sure.

I don't think it's typical (aside from OBGYN), but I don't know of any rule stating that they can't. We have some providers who are considered specialists, who occasionally bill for routine and non-specialty type services.

It doesn't happen often, though, because they are specialists - they went through a lot of extra schooling to become experts in a particular area; not so that they can see patients for things that are normally handled by primary care docs.
