We have patients do SOAPP, COMM, CESD-R, PMQ-R questionnaires & depending how they score, we make them do these so many times a year. What codes do I use for these questionaries? I was using G0444, 96136, & G0396 but having such a hard time getting these paid. Are there different codes I should use?
