I understand that the QW modifier is to be added to clia waved labs. My question is...is this only for regular Medicare? Or is this modifier used with Medicare HMO insurances as well, like United Healthcare Medicare or Blue Cross Blue Shield Medicare?

Qw modifier is only used with traditional Medicare
