Hi,

How would I know if our facility requires us to use the QW modifier on CLIA waived tests?

The easiest way to know if you need to use Modifer QW is if your facility has the CLIA certificate. If you do, then you will need to use the modifier QW. Just download their list of lab tests requiring the modifier. I have this list taped next to my computer for easy reference.

If your facility has a CLIA certificate, it will have a 10 digit certification number that is to be entered onto item line 23 of CMS- 1500 form when a lab code has the QW modifier on it.
