Does anyone know if E/M code is included in a 77263? Is it separately billable?

The E/M code is separately billable. I add the modifier 25 if I need to bill with the 77263. 77263 pertains to the physicians clinical treatment planning. Usually you will only have an E/M code with 77263 if it is an emergency and one leads to the other.
