Does the first date or last date go on the claim? CMS is denying. I don't see anything written for guideline.

Totally NOT my area, but wouldn't the first date AND the last date go on the claim? The claim form does have two date fields ("from" and "to").

It's been awhile since I billed Rad Onc but for 77435, I billed it with the date of service for the 5th fraction - or the last fraction if the SBRT course was less than 5 fractions. This was for physician based billing and you don't mention what provider/facility type you are in. If you haven't done so, check your MAC's website to see if they have any guidance. Our MAC - CGS - has a section on Radiation Oncology with various reference items.

The 77435 is billed with the last date of the treatment. As long as the MD treats the patient within the 5 factions, it will always get dated with the last fraction date.
