I have a physician that charges 77427 for 5 days of treatment. Can he also bill a E/M code?

So I have a question then. If the patient comes in for 5 days of consecutive treatment. What should we be coding from? The consult or should the provider be documenting something after every treatment? Just an FYI, I code on the hospital side. Thanks! Sarah
 
Physician services are documented by a weekly progress note. Attending physicians will examine the patient at least once per five fractions of treatment, and the code is 77427 (professional only, no hospital or facility charge). No other E&M code is allowed during the course of treatment or 90 days post RT.
