What would be the appropriate code(s) to use in this scenario;

Pt being treated for lung Ca. Note states, "Pt is receiving weekly chemotherapy in conjunction with her radiation therapy."

Can I code V58.0 with V58.11 if the pt is not being treated with chemo nor radiation on the date of the F/U visit? Or would I code V15.3 with V87.41?
 
If the patient is being seen in regular follow up and is still currently receiving chemo and radiation then the history of codes are not appropriate at this time, those you use when the therapies are completed. Also since this is not an encounter for chemo or radiation then the V58.xx codes are not appropriate. It sounds like this patient is still undergoing treatment for lung cancer therefore the code for the lung cancer is the correct code.
