need help in correct way to code for RFA,
LUMBAR MEDIAL BRANCH RADIOFREQUENCY ABLATION
UNDER FLUOROSCOPY
1) LEFT L3-L4, L4-L5 and L5-S1 medial branch radiofrequency ablation

would 64625 be billed for the l5-s1 or 64636?
it says not to bill 64625 with 64635

I am having similar problems billing the SRF. Per guidlines 64625 is the CPT code for that. Would love to know anyone else insight.

64636 should be used in conjunction with 64635.
