NUCLEAR MEDICINE LYMPHOSCINTIGRAPHY
NUCLEAR MEDICINE INTRAOPERATIVE LYMPHOSCINTIGRAPHY

INDICATION: Melanoma.

Radiopharmaceutical: 2.0 mCi technetium 99m sulfur colloid intradermal
0748 hours January 8, 2015.

Following the intradermal administration of the radiopharmaceutical in
3 divided doses around the periphery of left temporal lesion
conventional scintigraphy including transmission images, SPECT CT of
the head, neck and upper chest is performed. Scintigraphic, SPECT
tomographic images and SPECT CT fusion images are generated and
reviewed.

Activity related to injection sites is quite intense. Much less
intense migration of activity to areas at the left face and neck are
observed. There is a localized as follows:

Anterior inferior aspect deep left external auditory canal.

Inferior aspect shallow left external auditory canal, best localized
to the posterior superficial portion of left parotid gland.

2 lymph nodes within left upper neck, level of mandibular angle,
likely one each in level IIA and level IIb.

Unexpected activity residing within the confines of calvarium at the
middle cranial fossa. I would have to suspect this represents
artifact of patient motion and probably represents nodal accumulation
in the extracranial temporal fossa. Inspection of this region with
gamma probe encouraged.

IMPRESSION:

Positive migration of activity from injection sites at left temporal
scalp 2 sites of left neck, left parotid, region of the left external
auditory canal and perhaps left temporal fossa.
