Hi,
Should we charge separately for skin marking (liver biopsy) if it was done a week before the procedure itself or should it be bundled in the procedure and not be charged separately?

The second question is regarding bilateral breast biopsies. Can I bill them under one cpt with 2 units and a modifier LT and RT? Or 2 separate charges with LT and RT or one cpt with modifier 50?

For example;

or

Or
