Can anyone tell me if the doctor should read this using a transvaginal or a transabdominal approach?
Any advise is much appreciated.
 
The technique used by the sonographer is what determines whether it is transabdominal or endovaginal.

Frequently, both approaches are used by the sonographer, and if they are both documented by the radiologist, then both approaches are billable.
