We are in the beginnings of our first ever Medicare RADV audit. We are in the process of gathering our MR, but we are unsure how to name the charts for those that have more than one HCC in the MR. The instruction only say use one HCC in the name but then says you can have more then one HCC in the MR. Which HCC do you use? Just any of the HCC's that are in the MR? Any seasoned RADV survivors able to help?
