How do we accurately code for re-excision of a lesion when the original lesion has been removed and we are basically excising around a scar (typically where the original surgery had a positive margin or to re-excise a melanoma with larger margins). What do we use as length and width of the lesion at the time of the re-excision? Do we use the dimensions of the scar (e.g. 2.6 cm by zero cm) and then add the margins or do we use the original preo-op size of the lesion from the initial surgery with added margins?
