Wiki Readmit to SNF - new or subsequent?

Hi -

Quick question regarding readmit to a SNF, because I'm a little confused on the rules... If a patient is in a SNF, gets admitted to the hospital - under the 30 day threshold - and then gets readmitted, the first visit by palliative care is still a subsequent code, right? Not a new admit code.
