would you code this as a PTCA or an atherectomy. since they didn't mention extraction or removal I am leaning towards a PTCA.

Atherectomy - if your doctor documents it correctly. It can be used as a catheter to facililtate subsequent angioplasty or atherectomy, or it can be used as an atherectomy device itself. FDA approved the Bard Crosser CTO catheter as an atherectomy device (peripheral arteries) in July 2011.
 
so, what should I ask my physicians to document to make sure the crosser was used as an atherectomy device (besides the obvious "performed atherectomy")?
