When a patient requests to have a copy of their UB-04 or HCFA 1500 claim form for AFLAC, do we need to get a Medical Records Release filled out by the patient? I am getting mixed reviews saying that it is part of the medical record and others saying that it is a financial document and a release is not needed. I just want to be in compliance but not create extra work. Any input or website links would be great. Thank you.
 
Since the UB and 1500 claim forms contain PHI, they fall under HIPAA regulations and require the same handling as medical records - you should follow your office's or facility's medical records release policies for these the same as you would for medical records. Financial documents would be, for example, bills or statements that have no identifiable treatment information (e.g. CPT or ICD codes) included.
