Is a refraction inclusive in the surgical package? I had found it years ago in the surgical guidelines for the chapter that the 1 refraction was included in the package, but I can not find it now. Please help!

Thweoretically, you could bill the patient for each post op refraction. However, in this age of specialty IOLs, most eye docs don't do that because they're basically using the information to determine how accurate their IOL selection was, especially when the patient has paid significant extra fees out of pocket for these lenses. Foe patients who had monofocal IOLs implanted, I wouldn't charge for refraction during post op visits, except for the last PO visit, at which time I would give them a final RX to update their eyeglasses.
