Hello,
I have a visit where the parent of the child refused the HPV vaccine, (no reason given) but allowed other vaccines to be administered. Do I need to code the refusal of the HPV vaccine? Thanks ;)

As far as I know the HPV vaccine is an optional vaccine. not like the measles vaccine. So I do not feel there should be any reason to indicate the parent declining something optional. Correct me if this is incorrect.

We do not "code" the refusal in any way. However the providers do document the refusal in the progress notes for liability reasons. (There have been cases of patients getting a vaccine-preventable disease and the parents filing a lawsuit against the provider because they were not "offered" the vaccine... we want to be able to show we offered and it was the parent's or patient's decision not to get it.)

I understand, documentation by the provider in the chart note and maybe having the patient/parent sign a form indicating the vaccine, date and refusal, should be sufficient.
