Can you please tell me whether to code V57.1 as Pdx while doing physical therapy for a patient with hemiplegia due to old CVA under home health services... any guidelines to be applied for coding home health services such as Nursing care, physical therapy, occupational therapy etc..?

Yes the V57.1 is first listed only allowed and is to be used for all rehab services. This is in your coding guidelines in the general guidelines section number 15 titled encounters for rehabilitation.
