If a physian fills out paperwork (ie. wic form, disability papers) can it be billed and reimbursed? How about when you provide this in a federally qualified health clinic? What code would we bill?
 
I work in an FQHC, non profit community health clinic. We bill school physicals or disability visits with the v70.3 - Other medical examination for administrative purposes. But the provider must also document a note in our EMR. If the patient is requesting disability then the provider should have at least done a problem focused exam and review of old records....

Follow up question

Let's say the provider says we do not need to see the patient. The patient only needs a form filled out. I am telling them it's not medically necessary but someone else is telling them they can make a visit out of it.
