Patient has injury - is billing through health insurance-Patient requests our doc fills out short term disability form and provides us with form.

Would you have patient sign a release to give disability co. PHI since it is different ins co. than her health ins???? the disability co. also is calling us for information and I am very reluctant to give out info with no release.
