Removal of a Moss Feeding Tube?

My physician's OP Note Reads as follows: Patient was done as an outpatient in the hospital setting. Physician says: Fluid was removed with a syringe until no more could be removed and then gentle pulling and twisting allowed the tube to be removed completetely. A dressing was applied to the site. Is this just an E/M visit code or is this an unlisted code or is there a CPT code for removal of a Moss Feeding Tube?
