Hello, does anyone know what CPT code is used for a removal of a thrombosed AV graft en bloc? I am looking at 35903; however, this code is for excision of an infected graft extremity. In this case it was removed due to arm pain and graft was thrombosed

Body of OP note:

The patient was prepped and draped in standard fashion. Proximal and distal control were obtained through two separate
2 cm incisions. Umbilical tapes were used to control the inflow and the outflow limbs. Next the length of the access was dissected
out along its course. This was accomplished by dissecting down to the access where it was then freed circumferentially. The
entire thrombosed access was then removed en bloc. The brachial artery then was reconstructed at the prior anastomosis with a
vein patch. Hemostasis was achieved. The excess skin was then resected the incision was closed in several layers with 4-0 vicryl.
The patient's skin was cleaned, dried and sterile dressings placed. The patient was then transported to the PACU in stable
condition.

35903 is included anastomosis removal, and vein patch for closure of brachial artery is required, so no additional code for vein patch.

36832 - revision AV fistula, graft is excised, but fistula is not removed.
