I have a denial that I am trying to fix.
Dr Billed out 11404 at 2 units. 12032 at 2 units and 99354. now after reading the progress notes I see immediately that the the 11404 should be a 11403 and it should be listed twice not at 2 units. ( he removed 2 cysts from the back) and for repair he has 12032 listed at 2 units, and then the add on code, so I am slightly confused on how this needs to be correctly billed.. I know or at least have learned that it does not go out with units, but as for modifiers, what would be the best?

your repair codes (length ) in the same anatomical must be added together. not sure why you are billing 99354.
