UHC doesn't like either CPT that we billed on the attached op note stating that neither 20680 nor 26011 are supported. Would anyone be willing to look at this op note and tell me what CPT's you would have billed. I really feel that the hardware removal (20680) is documented. Would you agree with 26011 for the I&D? Thank you,

Hello,
this wasn't and I&D but a debridement of bone with bone infection and necrosis. check your path to see if osteomyelitis.
for the retained hardware dx make sure you have a T code for painful ortho prosthesis (implant) for the 20680.
