I have a case I have not seen before, but I'm SURE it's not the first time in history this has happened. Patient has had 3 previous two-piece inflatable penile implants, each one has had a malfunction of one of the cylinders. This time he opted to replace it with the 3-piece implant. Here's my question: Is it better to code the removal at 54415 and insertion at 54405? AUA allows it, but I'm not sure his Medicare replacement will see it that way. 54410 is what was authorized, but I thought that was taking out and replacing the same type of implant. Am I just overthinking it and is the 54410 the way to go? Thank you!
 
After thinking about it ALL morning, that's what I ended up doing, because there's nothing in the code description anywhere that said it MUST be the same type of implant put in as taken out.
