Wiki Removing High Risk Status

I have been told 2 different things about what it takes for a patient who has been high risk to be removed from that high risk category. I've been told that the patient must have no less than 3 consecutive negative pap results, and I've also been told that the patient must have 5 years of negative pap results. I can't find any documentation as to what the true guideline is for this either online or in my coding books. Can anyone point me in the right direction? This patient has a BCBS commercial plan. Thanks!
