so the doc removed a skin lesion .6 x .6cm and coded it 11602 w/232.5. he has since called the patient back in and removed the margins 1.5 x 2.5cm.

does someone please no how to code the second visit???

You would code for a malignant excision and add a 58 modifier. You determine the size of the excision by using the size of the origianl lesion. Final repair should also be coded with 58 modifier as well. The diagnosis should be the same as the reason for re-excision which is malignancy no matter what the path report comes back.
