Repaeat Colonoscopy

Any help with this will be appreciated . Patient had colonoscopy for history of polyps and physician found an large polyp in Cecum which was removed but now the physician wants to repeat procedure a 6 months later due to size of polyp/making sure no fragments remain. This is a Medicare patient so I was wondering what indication I would use? Z86.010 or D12.0 or would I code just the findings. Since the Time frame is less than 2 years i don't believe Medicare will cover this if i use Z86.010 as primary diagnosis.

I would use D12.0 for the follow up procedure indication since that is the reason he is coming back in.
