Look at 42107 - Excision , lesion of palate with local flap closure. Don't let the fistula closure distract you, your surgeon in the process of removing this lesion created the fistula defect and had to closed it using a rotational flap. The excision of the lesion and the type of closure is your primary procedure. If they needed to use a graft to close you could bill for that as well.
