I have run into an issue where we are having trouble with reimbursement and I am not sure now how to code this.

Pt has a discontinued Screening Colonoscopy in the AM and it is coded as a 45378 -53 Professional and 45378- 74 Facility. we get reimbursed for that.

Same pt returns for the PM session and now has a repeat procedure with another MD and a snare polpectomy is done. This is getting denied with modifiers 77.

I need help with this pleasde any ideas?

Modifier 77 stated that the prior procedure was repeated...the am procedure was a diagnostic colonscopy and the pm procedure was a colonoscopy with a snare polypectomy....two different procedures
