If a physician has pt come back for a wart removal 2 wks after the first one was done and in dictation says might nd to be treated 3 times after that would you use modifier 76?
 
We use 17110 as the code for removing warts via cryo. There are times when it takes 2 - 3 visits for this to be successful. I generally do not use the 76 modifier and have not had problems getting reimbursed.
