Wiki Reporting or coding BMI without documentation of obesity?

I need one clarification regarding BMI code.I know it should be reported as secondary code along with the diagnisis of obesity or morbid obesity codes because BMI is just the number may be entered by others just before the visit other than rendering provider. My doubt is can we report BMI status code Z68.41 code if provider address this in assessment with weight reducing plans without documentation of obesity or morbid obesity. He addressed as it is by documenting Z68.41 code in assessment and plan but not mentioned any diagnosis of obesity. Do we still need documentation of overweight,obesity and morbid obesity documentation to report Z68.41 code?
