Anyone know how I would report an add-on code such as 11045? Do I list the units on separate claim lines or on claim line with multiple units?? Thanks for any help you can give, I have been searching for this answer all morning!
 
Unfortunately, the amount of units are left up to the carriers. I know for MCR MA, GA require them to be on one line up to 4 units.
