Wiki Reporting saline for lipoatrophy?

I am searching for assistance in being able to file and injection of saline when 11900 is used to administer saline due to lipoatrophy (buttocks)? This was after they patient received a steriod injection 6 mo prior. To me this is cosmetic. I am new to dermatology. The provider has filed only the 11900. I do not think I can even file a 11900 without the saline combination. Do I tell the provider that services cannot be filed for this patient since this does not warrant medical necessity? Would a saline injection ever warrant medical necessity in a dermatology setting? Thank you for any advice that can be given. ::)
