HI - I dont officially deal with this but one of our nurses ask me if I heard of a requirement from Medicare that a insulin log must be keep for a patient needing to test more often then 3 times a day and given to pharmacy - in past she stated they've just provided a letter certifying medical nec.

Any input is always appreciated!!
