Wiki Residents do they have to sign the chart?

I bill for the physicians for an ER group and recently I have noticed that we have residents signing the charts along with the ER doctors. The residents are not doing procedures. They are just doing the History and my docs are doing the examination. I am unsure if its okay for the residents to sign the charts. I am trying to find out if we are considered a medicare teaching facility, If residents are doing the history only and we are not considered a Medicare teaching facility do we still need to follow the guidelines were the doctor states he performed the service and we present for critical portions of the exam?
Is there a difference between a "approved GME program" and a "medicare teaching facility" ? Any help would be greatly appreciated!
